If you are traveling in a developing nation, you should find an insurance company that charters its own airplanes. Turns out one of the only significant medical costs abroad in he third world is if you have an emergency and need to be air-lifted out. Air evacuations can be costly, apparently, and if your insurance company doesn't have its own planes, it can take a long time for them to find one to use.
